Diabetes mellitus (DM), görülme sıklığı ve komplikasyonlarından dolayı önemli bir sağlık sorunudur.
DM’nin kronik komplikasyonlarının etkileri çoğunlukla alt ekstremitelerde görülür. Diyabetik ayak
lezyonları önlenebilir, fakat meydana geldiklerinde ciddi morbiditeye ve hatta amputasyon ve
mortaliteye neden olabilirler. Tedavinin ana hedefi ampütasyonu önlemektir. Ülserin klinik olarak
iyi tanımlanması ve bu amaca ulaşmak için uygun tedavi yaklaşımının belirlenmesi gerekir.
Bu çalışmanın amacı, diyabetik ayak hastalığı ve tedavisinde kullanılan ilaçları incelemek ve
değerlendirmektir.
Diyabet hastalarının %15’inde ayaklarında ülser görülmekte ve ortalama olarak bu hastaların
%25’inde amputasyon gerekmektedir. Diyabetik ayak ülserinin iyileşme süreci, kronik inflamasyon,
fibroblast fonksiyonundaki bozukluklar, zayıf anjiyogenez ve hücre göçü eksikliği gibi faktörler
tarafından engellenmektedir.
Diyabetik ayak ülserlerinde uygulanan adjuvan tedavi yöntemleri belirli bir seviyede amputasyon
riskini azaltmasına rağmen, bu yöntemler yüksek iyileşme sağlayamamaktadır.
Öte yandan, epidermal büyüme faktörü kullanan tedavi yöntemi önemli bir iyileşme yüzdesine
ve daha az amputasyon riskine sahiptir. Tedavi yöntemlerinin yanı sıra, ayak bakımı yapmak ve
ülserin gelişmesini önlemek için gerekli eğitimi almak son derece önemlidir.
Diabetes mellitus (DM) is an important health problem due to its incidence and complications. The
effects of chronic complications of DM are mostly seen in lower extremities. Diabetic foot lesions
can be prevented, but when they occur, they can be cause serious morbidity and even amputation
and mortality. The major target of treatment is to prevent amputation. It is necessary to define
clinically of the ulcer and to set the proper approach of treatment for getting this aim. The aim of
this study is to examine and to evaluate diabetic foot disease and drugs used in its treatment.
In 15% of diabetes patients, it is seen ulcer on their feet and, on average, in 25% of these patients
require amputation. The healing process of diabetic foot ulcer is inhibited by factors such as
chronic inflammation, defects in fibroblast function, poor angiogenesis and lack of cell migration.
Although adjuvant treatment methods applied in diabetic foot ulcers reduce the risk of amputation
at a certain level, these methods cannot provide high recovery. On the other hand, the treatment
method using epidermal growth factor has a considerable healing percentage and less amputation
risk. Apart from the treatment methods, it is extremely important to perform foot care and to take
the necessary training to prevent the development of the ulcer.
